Custom Forms: Requiring a File Upload

Forms can be set up to allow the patient to upload files. This is especially handy if you are trying to collect a copy of the patient's insurance card, driver's license, or even a headshot. 

Step 1: Start by navigating to your settings.

Step 2: Proceed to the Forms section under Patient.

Step 3: Either create a new form or edit an existing form.

Step 4: Add an element and select the File Upload option.

Step 5: Assign a title and a description to your image upload element. If necessary, make the field required. After you've done this, hit Save.

Step 6: Don't forget to publish your form once you're done editing. Preview the form by clicking the Preview button.

When patients receive a form with the upload element, they'll see the description and have the option to upload a file. The supported file types are HEIC, JPEG, and PNG.

After uploading their file, patients can adjust the scale and/or rotate their image.

Once they're satisfied with their image, they'll click Confirm to see their image uploaded to the form. To finish, they'll hit Submit.
