Using the Form Structure Elements in Custom Forms.

In this article and video, you'll learn how to structure your form by grouping questions, showing them conditionally, and adding text blocks using the Structure elements.

There are two options available in the form structure: Sections & Text Block.

The first option is the sections. This allows you to group a set of questions.

This option requires a title.

You're able to conditionally show this section.

You can choose the conditions of age and gender of your patients. For this example, we'll use this section for females aged 16 and older.

After entering the details, click save.

Now, use the question elements to create the questions you'd like to include in this section.

Once you've created the questions for this section, click the drop-down arrow to open your section.

Then, left-click and drag your questions into the section.

The next form element is the Text block. This element allows you to include a block of written content into your form for the patients to read.

Title your text block and write the body of your text block.

Once you've filled out your text block, click save.

You can then left-click and drag it onto your form wherever you choose.

Now that you have your form created with your text block and section of questions, click Preview to review your form.

If you're satisfied with your form, click Publish.

Congratulations! You've now published a form using structure elements.
