Custom Forms: Manually Attaching and Sending Forms

In this article & video, you'll learn how to manually attach forms to a patient's appointment and how to send these forms to the patient.

When you create an appointment, the system automatically attaches the necessary forms if the CDT code on the appointment matches automation settings or the CDT codes set on the form.

Note: If you have variables on your forms for appointment date, time, etc., you must attach the form in the patient snapshot under the forms section, as shown in the images below.

If you attach forms using the Forms navigation, this does not pull appointment data, as the form is just being attached to the patient using this option. 

To manually attach forms, click on the patient's appointment.

In the patient snapshot, scroll down towards the bottom where you'll find the forms.

Click on the plus icon.

A list of all the forms will appear. Select the forms you want to attach to the appointment.

After selecting the forms, click on Attach.

The system will automatically send out the forms based on the incomplete forms automations you've set up in your communications tab.

If you prefer to manually send the forms, select each form so they're highlighted in blue.

Click on the three dots.

You'll have the option to send the forms to a kiosk if the patient is in the practice. The kiosk is an iPad or tablet that patients use to fill out consent forms.

You can print the forms for physical signing.

You can click Send to send them via text to your patient.

The recipient's information will appear. If there are multiple family members, you can select which patient to send the forms to.

You can edit the pre-populated message as needed. Use the forward slash key ( / ) for any variables you'd like to use, and be sure to include the forms link.

Click on Send when complete.
